How Shoulder Dystocia Becomes a Malpractice Claim

The first involves the period before delivery: an OB-GYN who identified risk factors for shoulder dystocia but didn’t discuss or plan for an alternative delivery approach may have fallen below the standard of care. When shoulder dystocia is recognized, obstetric teams are trained to use maneuvers such as the McRoberts maneuver (hyperflexing the mother’s thighs toward her abdomen to open the pelvis) and suprapubic pressure (applied externally to dislodge the shoulder). Applying excessive traction or downward force instead of these recognized responses is where avoidable injury often occurs. OB-GYNs, nurses, and midwives each carry distinct duties during the emergency, including timely communication and escalation to a cesarean delivery when necessary.
